  • • Proof of residence (signed lease, deed, mortgage, most recent utility bill addressed to the parent/guardian at the residence, or the order to turn on utilities) 

    • Complete address of the previous school attended 
      (applicable for grades 1 - 12)

    • Most recent report card or current transcript
      (applicable for grades 1 - 12)

    • Business License Number, if applicable
      (for local business owners)

    Separate submissions of this form are required for
    each Non-Resident student enrollment application.
